Change Request

A change request is a formal request to make a change to a baselined configuration item. A request for change may be triggered by:

  • Identification of software deficiencies
  • New or changed requirements
  • Design changes
  • Hardware changes
  • Cost saving initiatives
  • Schedule accommodation.

Typical Information Provided in a Change Request

Item Comment
Contract details Provide customer name and contract number
System name  
Originator details Identify the change request originator. Provide originator name, signature, organisation and contact numbers (e.g. phone, facsimile, e-mail)
Submittal date Provide the date the change request form was completed
Change request title A sentence summarising the change
Change description A description of the required change may include functions affected and the reasons for change. If the change is substantial provide or reference supporting documentation
CIs impacted The names and identifiers of configuration items impacted together with the severity of the impact [major | moderate | minor]
System impact A broad assessment of the overall impact on system scope, cost, quality and/or schedule expressed as [major | moderate | minor]
Related change requests Identify any existing change requests that deal with the same or related issues
Priority Expressed as high, medium or low
Attachments Provide supporting information as required
