Change Log, Issue log & Change Request

HI,

Can anyone explain the exact difference between Change Request & Change log ? Definition of "Manage Stakeholder engagement" - Communicating and working with stakeholders to meet their needs and address any issues as they occur in the project From the definition it can be said that PM also solve issues if any. then why Issue Log is not used as Input to "Manage Stakeholder engagement" ?

please help

If we want any change, we raise change request but after a change request is approved or rejected, result is documented in change log.
If any issues are observed in manage stakeholder engagement, it is documented in issue log(outout of the process). Issue log is then used as input in control stakeholder engagement process to resolve them.
